Refund Policy

In the event that you wish to request a refund on an unscheduled Assessment that has been paid for online, Wellpoint will refund the entire amount of the assessment less a $50 administration fee. For refunds on scheduled assessments, the amount of the refund will be the amount of the assessment less a $250 administration fee if greater than forty eight (48) hours notice before the scheduled appointment is given. Any less than forty eight hours notice will result in the total amount of fees being non-refundable.
