Program Details

We provide you with a coordinated approach to your healthcare, beginning with a complete, 360º assessment of your health through a physician consultation, a physical exam and a full spectrum of diagnostic tests followed by a plan for your care throughout the year. The Health Blueprint Platinum is included with the Wellcare Program and provides the basis for the Health Blueprint (which lays out your current baseline health status rating).

Should the assessment indicate any area that requires attention, a designated Wellpoint case manager will coordinate and direct your care. Taking direction from the assessing physician, the Wellpoint case manager will work to maximize coordinated efficient health services, including referrals for specialists or diagnostic consultations.

Over the course of the year, if you find yourself thrust into the healthcare system through illness, injury or accident, one call to Wellpoint brings us to your side. We act as your advocate within the medical system, facilitate your care, and keep lines of communication open to ensure peace of mind for you and those that care for you.
